During National Case Management Week, we celebrated the dedication of care managers at Southern Tier Connect by sharing shout-outs of gratitude from families, members, and staff for all the work care managers do. Care managers collaborate with families and professionals and advocate for services to coordinate care for the people they serve. During the week, Executive Director Jessica Colon and Director of Care Management and Compliance Carey Peters visited Southern Tier Connect locations to personally thank the fantastic care managers at STC. Throughout the week, Jessica and Carey shared breakfast with staff in Cortland and Norwich and lunch with staff in Binghamton and Oneonta; they also delivered cookies to every office.
Read quotes below from families, staff, and members thanking care managers for their hard work and dedication.
“Plain and simple, Betty is a superhero and always has been for us. Whenever we have a question or advocacy issue, she is right there with us (even when she may not completely agree with my mom’s rants) and supports us every step of the way. She also dives right into helping us obtain services. Most recently, Mason wanted to return to horseback riding lessons, and we were really having a hard time finding a suitable place. Betty constantly sent me places to check into, and eventually, we found a great one! Thanks for being amazing, Betty!”

“I would like to thank and recognize the phenomenal group of staff I have the immense privilege of supervising: Elayne, Kara, Holly, Kristina, Shannon, and Natalie; Britt, who has given me the opportunity to be a Social Work Field Instructor for the first time; and all of the care managers I have had the pleasure of supervising in the past: Carl, Matt, Nick, Jamie, and Kim. You each bring something unique to the table, which makes STC stronger as an organization. You each show up every day and work tirelessly to help the members you support. I know it’s not often easy. Care management is a BIG job, and it takes a special person to do it, but you all have that something special. You are superstars!!! You are sincerely appreciated! Thank you, thank you, thank you for all you do!”

STC member Ryan gave a shout-out to his care manager, Dorothy, for “everything that she does.” Dorothy visits Ryan at home and in the community, where he participates in many activities.
Ryan likes that she is “very efficient and helps with anything he needs” and takes the time to listen to his many interests, which “means so much” to Ryan.
“She is a very sweet and sincere individual,” said Ryan.
Thank you so much for sharing your thoughts about your care manager, Ryan! And thank you, Dorothy, for all the support you continue to provide to our members.
The Tredo family reached out to us to share a few words about Care Manager Mimi Tyree for National Case Management Week: “Mimi Tyree has been a godsend to our family, working tirelessly to improve our lives and get the services my son needs. Her friendly smile and kind words are always there to let us know that we will get through whatever comes our way. She is always contacting us to make sure we have everything we need. And always responds quickly whenever we call. I don’t know what we would do without her.”
