Know Your Rights

At Southern Tier Connect, we provide care coordination from a familiar face in a friendly place. We are a small care coordination organization (CCO) with service areas in Central and Southern New York State that cover 14 counties, with office locations in your town or village. Small is our approach to providing your care because we understand that the best care comes from a relationship you can trust in a place of comfort. Small is also an attitude. We are responsive and innovative; we are always ready to answer your questions and help you meet your goals.

Most of all, at STC, small is a promise. We care about your rights as a person with a disability. Every person should be given respect and dignity regardless of race, religion, national origin, age, sex, gender identity or expression, ethnic background, sexual orientation, or developmental or other disability. You have the same fundamental rights as anyone else. These rights include the right to employment, the right to privacy, the right to practice your religion or beliefs, the right to express yourself, and the right to choose the relationships you want to have. Finally, you have the right to be your own person and express your independence.
