During National Case Management Week, we celebrated the dedication of care managers at Southern Tier Connect by sharing shout-outs of gratitude from families, members, and staff for all the work care managers do. Care managers collaborate with families and professionals and advocate for services to coordinate care for the people they serve. During the week, Executive Director Jessica Colon and Director of Care Management and Compliance Carey Peters visited Southern Tier Connect locations to personally thank the fantastic care managers at STC. Throughout the week, Jessica and Carey shared breakfast with staff in Cortland and Norwich and lunch with staff in Binghamton and Oneonta; they also delivered cookies to every office.

STC member Ryan gave a shout-out to his care manager, Dorothy, for “everything that she does.” Dorothy visits Ryan at home and in the community, where he participates in many activities.

Ryan likes that she is “very efficient and helps with anything he needs” and takes the time to listen to his many interests, which “means so much” to Ryan.

“She is a very sweet and sincere individual,” said Ryan.
